This position serves in a front-line role for GMCC, interacting with patients and ensuring the delivery of quality care in compliance with the standards, expectations and values of our organization. 24-26 hours weekly.
essential functions
· Escorts patients to triage, examination rooms and front desk for check out.
· Gathers patient information at the start of the visit to include vital signs, oral assessments, visual acuity, etc. as required. Assesses presenting problems and indicators of life-threatening conditions.
· Performs medication and allergy reconciliation at each patient visit.
· Ensures timely, thorough documentation of all patient interactions in accordance with medical-legal standards and Clinic policies.
· Performs routine patient procedures i.e. lab tests, vaccinations, EKG’s, nebulizer treatments, etc.
· Assists providers during patient examinations and treatment, i.e. wound care, I&D, suture removal, etc.
· Responds to routine patient inquiries and/or telephone calls. Determines initial assessment of patient condition and determines appropriate plan of care according to established guidelines.
· Provides patient education and monitors patient compliance.
· Facilitates patient referrals
· Provides orientation and mentors new clinical team members and volunteers.
· Accurately tracks and reports data as directed.
· Inventories and maintains stock of medical supplies and ensures exam rooms are properly equipped.
· Maintains medical equipment as appropriate.
· Actively participates in staff and committee meetings as necessary and appropriate.
· Complies with HIPAA (Health Insurance Portability and Accountability Act) and OSHA (Occupational Safety and Health Administration) training requirements and protocols.

· Must be a Certified Medical Assistant (CMA).
· Proficient at performing basic medical/technical procedures.
· Experience using an Electronic Medical Record (EMR) preferred
· Phlebotomy experience preferred.

· Provide guidance, support, and monitor clinical staff, manage clinical flow, and direct nursing staff in day-to-day tasks
· Coordinate patient care with physicians, clinical staff, front-end coordinator, and case manager
· Address Patient Needs or Complaints and respond to routine patient inquiries and/or telephone calls
· Create Staffing Schedules, nursing assignments, and work collaboratively with the Director of Patient Care to address staffing needs/concerns

This position is a vital leadership role at GMCC, primarily responsible for planning, organizing, directing, coordinating and evaluating patient care services provided by the Gloucester Mathews Care Clinic. Will partner with senior management, working under the direction of the Executive Director and working collaboratively with the Physician Practice Manager and Medical Director. Will help provide leadership to the entire GMCC organization.

Responsible for maintaining sufficient, qualified resources with which to operate
Works closely with employed and volunteer providers relative to schedules, requests, and the execution of quality patient care
Attends and actively participates in appropriate Executive, VAFCC, Williamsburg Health Foundation, and team meetings
Manages and mentors clinical and front desk team members
Oversees patient navigation to include referrals, hospitalizations and emergency care
Directs, trains and assists volunteers with referrals, procurement of pertinent patient records, screening/triage of new patients and chart preparations
Administers and trains externs, interns, students and clinical volunteers
Monitors patient compliance
Works collaboratively with the Executive Director and the Data Coordinator to ensure data standardization, data entry and reporting goals are met
Responsible for clinical risk management
Manages clinical inventory/equipment/supplies
Serves as the patient advocate and patient navigator
Develops and implements clinical policies and procedures
Complies with and ensures adherence to HIPAA and OSHA training requirements, clinical guidelines and protocols
Monitors budget for outside pharmacy patient medications
Monitors and evaluates team member’s performance
Works with Medical Director and Practice Manager to ensure that clinic continues to remain up-to-date with the most recent medical guidelines
Works with medical Director and Practice Manager to make sure Chart Audits and Peer Reviews are completed
Works with billing provider to ensure that billing issues are identified and handled appropriately

Registered Nurse with significant management experience in a clinical practice
Valid drivers’ license with a good driving record
Demonstrated record of leadership abilities
Knowledge of CMS guidelines and billing or coding experience
Knowledge of Human Resources principles and practices
